01 Purpose

Crystal Ball is a medical underwriting query platform for licensed insurance professionals only. It does not provide medical or legal advice, and it does not make binding carrier commitments. Outputs are informational aids to your professional judgment, not guarantees of any carrier decision.

02 Eligibility

To use the platform you must be at least 18 years old, hold a valid insurance license, and keep your account credentials confidential. We reserve the right to terminate accounts that do not meet these requirements.

03 Acceptable use

Permitted activities include querying carrier guidelines, running scenario analysis with de-identified data, and collaborating with your team. The following are prohibited:

• Any unlawful use of the platform.
• Scraping or bulk-extracting data.
• Sharing credentials between users.
• Misrepresenting platform results to clients or carriers.

04 Client data

You warrant that you have the necessary authorization to use any client health information you enter, and that you will comply with applicable state insurance regulations and federal privacy laws. We provide technical safeguards, but you remain responsible for your own HIPAA compliance.

05 Intellectual property

Crystal Ball owns the platform software and the aggregated carrier data. You retain ownership of the content you create, but you may not redistribute carrier guidelines without consent.

06 Disclaimers

The platform is provided "as is" without warranties of merchantability or fitness for a particular purpose.

07 Limitation of liability

Our total liability is limited to the greater of the fees you paid in the preceding 12 months or $100 USD.

08 Indemnification

You agree to defend and indemnify Crystal Ball against claims arising from your violations of these terms or your misrepresentation of platform results.

09 Fees

Subscriptions renew automatically and are non-refundable except as set out in our refund policy.

10 Termination

Either party may terminate at any time. We may suspend access immediately in the event of a violation of these terms.

11 Disputes

These terms are governed by the laws of Louisiana. Disputes are resolved through binding arbitration in Bossier City, Louisiana.
